Aquifer is committed to providing contemporary teaching in all aspects of healthcare.

Our team of medical educators has determined that Aquifer Medical Home cases no longer represent the complex topics critical to the effective delivery of care within the medical home model.

Therefore, Aquifer Medical Home will retire permanently on June 30, 2024.

Suggested Next Steps:

  • Update your curriculum and syllabus as needed
  • Share this news with students or faculty as appropriate
  • Download any course reports with student log data related to use of this course before June 30, 2024.
